REQUIREMENTS FOR REGISTRATION OF BIRTHS
Parents of Filipino babies
born in China should first secure the translated, notarized and
authenticated birth certificate from the following:

Once this is secured,
present the following to the Consulate General:
1. Original
and two (2) copies of the notarized and authenticated birth certificate.
The baby’s footprint should be imprinted on the authentication page of the
Shanghai Foreign Affairs Office.
2. Original
and two (2) copies of the notarized and authenticated Marriage Certificate
or Affidavit of Paternity (if not married)
A. If
married in China, please have the marriage certificate notarized and
authenticated by the concerned Chinese offices.
B. If
married in the Philippines, secure the Marriage Certificate from the National
Census and Statistics Office and have it authenticated by the Department of
Foreign Affairs. (Authentication Section)
3. Three (3) copies of Passports (data and back page) of both parents.
4. Duly accomplished Report of Birth
forms. Original and two photocopies.
FOR* REPORT OF BIRTH FILED MORE THAN 12
MONTHS AFTER THE CHILD'S BIRTH, AN ORIGINAL AND TWO COPIES OF NOTARIZED
AFFIDAVIT OF DELAYED REGISTRATION MUST ALSO BE SUBMITTED.

PARENTS OF A FILIPINO BABY BORN IN CHINA MAY, AT THE SAME
TIME, APPLY FOR HIS/HER PHILIPPINE PASSPORT. REQUIREMENTS ARE AS FOLLOWS
